The purpose of this paper is to demonstrate the ability and understanding to create an effective termination plan within a particular setting and population. The following topics will be used in illustrating the components and aspects of a termination of practice plan: an overview of the practicum, including the setting, population, and specific duties within the practicum; the termination plan, including specific interventions and timelines for completion; and a conclusion to summarize the knowledge gained in completing a termination of practice intervention plan.
Overview of Practicum.
Comtrea is a community-based agency that provides a comprehensive range of mental health services to the residents of Jefferson County. One of the services provided by Comtrea is the Day Treatment Program (DTP). The DTP serves approximately 20-25 mentally ill adults who reside in boarding homes, nursing homes, and private residences throughout the area. The program provides an environment in which the clients can optimize their independent living and socialization skill in order to improve their quality of life. The daily activities I perform include: social interaction with the clients; assisting clients in completing their daily life skill; assisting in the facilitation of education groups and current events discussions; charting each clients participation in groups, their daily activities, and their physical appearance; and, on designated days, accompanying staff and clients on community integration trips. The reason for termination with the agency and program will be due to the completion of my practicum.

Intervention: 1) Notification of my approximate final date was announced with the initial start date. I will begin to remind clients of my termination with the program, and reasons for doing so, throughout the remainder of the semester. One month prior to my departure, I will begin reminding the clients that my final date will be April 21, 2004.
