Terry is currently enrolled in the 9th grade at South Floyd High School where he is .
Terry is a 14 year old, white male of seemingly .
average height and weight with sandy brown hair and green eyes. He appeared .
appropriately dressed and exhibited adequate hygiene. According to vision and hearing .
screenings conducted at the school, no difficulties were noted for vision, and .
speech/language/communication is average. According to school records, Terry's .
attendance was indicated as very good. Terry lives with his grandmother and his father. .
According to Terry's teachers he has difficultly keeping up with his peers .
academically. Terry's teachers also indicate that he has difficultly getting along .
with others and has a problem with following directions, as well as difficulty completing .
his assignments.
Behavior Information.
The day of testing there were no observable physical disabilities. During the .
evaluation, Terry used his right hand and his behavior was marked as normal, with his .
body movements indicating no gross or fine motor impairments. Terry appeared secure .
about test responses and seemed to present adequate self-confidence. Terry appeared .
friendly and cooperative and communicated freely with the examiner, demonstrating no .
speech and language difficulties. Terry appeared interested in the testing activities and .
displayed good attention and concentration during the evaluation. Terry displayed good .
eye contact with the examiner, and attempted all task presented in the testing situation. .
The results of the test are judged to be a fair representation of Terry's abilities, and fairly .
consistent with Terry's classroom performance.
